Vegetarian and vegan nutrition: how to support 5 key biomarkers

You have a result in front of you. Maybe active vitamin B12 is lower than expected despite a supplement. Ferritin is drifting down even though lentils are on the menu. Or the Omega-3 Index does not match the healthy routine you thought you had built. The next step is not to add every supplement at once. It is to make one sensible change, repeat it consistently and see how your body responds.
Part one of this series explained five blood markers worth knowing on a vegetarian or vegan diet. This second guide is about action. It covers the practical choices that may support active B12, ferritin, transferrin saturation, the Omega-3 Index and selenium in full blood.
No meal plan can guarantee a specific blood result. Absorption, blood loss, medication, inflammation and consistency all change the outcome. The useful pattern is simple: make a clear change, follow it steadily, then interpret a repeat result in context.
Vegetarian and vegan nutrition works best as a system
Long lists of ideal foods are easy to save and hard to live by. Build a few repeatable anchors instead: a reliable B12 routine, an iron-friendly meal, a clear distinction between plant alpha-linolenic acid, or ALA, and the long-chain forms reflected in the Omega-3 Index, plus a selenium plan that does not depend on guessing the content of one nut.
For a fully vegan diet, vitamin B12 is the exception to a food-first rule. The German Nutrition Society recommends regular and reliable supplementation because ordinary plant foods do not provide a dependable source. Iron, selenium and omega-3 choices are more individual. Current German guidance combines a well-planned diet with targeted supplementation where needed, rather than assuming every plant-based eater needs the same stack (DGE, 2024).
Start with the marker that needs attention. Ask whether the source is missing, the routine is inconsistent, absorption could be improved or a label is too vague. This keeps the plan focused and makes a later retest easier to interpret.

Vitamin B12: build a routine you do not forget
For people who eat fully vegan, B12 is not the nutrient to leave to chance. Fermented foods, spirulina and ordinary seaweed products are not considered reliable replacements for a B12 supplement. Fortified foods can contribute, but the label matters. Check the amount per serving, how often you actually consume it and whether another product already contains B12.
The best format is usually the one you will take consistently. A daily product can sit next to brushing your teeth, while a less frequent product may suit a weekly reminder. Those schedules are not interchangeable. The DGE gives no universal dose for all vegans and recommends regular monitoring with adjustment when needed. In a randomised trial of 40 vegetarian and vegan adults with marginal B12 status, two different weekly sublingual regimens both improved B12-related markers over 12 weeks. The study supports consistency, but it does not create one dose for every person (Del Bo' et al., 2019).
If you eat vegetarian, eggs and dairy may provide some B12, but the amount depends on your pattern. Active B12, also called holo-transcobalamin, adds information on the fraction transported to cells. One result is not a diagnosis, but it can show whether the routine deserves a closer look.
Iron: pair plant sources strategically
Plant-based iron is found in lentils, beans, chickpeas, tofu, tempeh, seeds, whole grains and fortified cereals. This is mainly non-haem iron, the form found in plant foods, unlike haem iron from animal foods. Non-haem iron is more sensitive to the rest of the meal. Vitamin C can support absorption, while polyphenols in tea and coffee can reduce it when consumed at the same time.
You do not need to redesign every meal. Give one naturally iron-rich meal a vitamin C partner, such as lentil pasta with tomato and peppers, tofu with broccoli, or porridge with seeds and berries. If ferritin is low or trending down, move tea and coffee away from that meal rather than removing them entirely. In a controlled isotope study in young women, vitamin C increased iron absorption from a rice meal, while black tea reduced it. The size of the effect varied, but the direction was consistent in women with and without iron deficiency anaemia (Thankachan et al., 2008).
Build one iron-friendly meal
A simple pairing can be more useful than chasing a perfect food list.

Iron supplements are not a sensible default based on tiredness or diet label alone. Ferritin reflects stored iron, transferrin saturation adds context on circulating availability, and the full blood count shows whether red blood cell measures have shifted. Normal haemoglobin does not automatically mean iron stores are adequate. An unexpected pattern deserves medical interpretation.
Omega-3: connect the food source to the index
Omega-3 is a family of fatty acids. Ground flaxseed, chia seeds, walnuts and rapeseed oil mainly provide ALA, the plant-based form. Your body can convert some ALA into the longer-chain forms EPA and DHA, but the amount varies between people. ALA-rich foods remain valuable, yet eating them regularly does not predict an individual Omega-3 Index.
The Omega-3 Index measures EPA and DHA together in red blood cell membranes. Vegetarian Health reports this index and the separate HS Trans Index. EPA and DHA help explain what sits behind the Omega-3 Index, but they are not listed as individual results.
Algae oil can provide DHA, and some products also provide EPA, without relying on conversion from ALA. Check the amount per daily serving rather than the large “omega-3” number on the front of the pack. In a double-blind randomised trial, eight weeks of microalgae-derived DHA increased the Omega-3 Index in healthy vegetarians (Geppert et al., 2005). This does not mean everyone needs algae oil. It shows why a direct long-chain source can be easier to connect with a repeat index than ALA intake alone.
Selenium: avoid turning one food into a precise dose
Selenium is the hardest of these nutrients to estimate from a food diary because the content of plant foods depends strongly on the soil and growing region. In Germany and much of Europe, plant foods are often lower in selenium than the same food grown elsewhere. Useful plant sources can include legumes, mushrooms, asparagus, cabbage and onion vegetables, but food tables cannot tell you exactly what your serving contained.
Brazil nuts are the obvious shortcut, and also the reason restraint matters. Their selenium content can vary widely. DGE guidance advises limiting them to no more than two per day, partly because they can also accumulate radium. Treating a handful as a precisely dosed supplement is therefore a poor strategy. A German study comparing 54 vegetarian participants with 53 omnivores found lower total serum selenium and selenoprotein P in the vegetarian group, while another functional marker did not differ. The study is a reminder that both intake and marker choice affect the picture (Hoeflich et al., 2010).
Before adding selenium, total the amount across your multivitamin, mineral blend and any standalone product. The European Food Safety Authority sets an adult upper intake level of 255 micrograms per day and identifies high-dose supplements and regular Brazil-nut intake as the main ways adults may exceed it (EFSA, 2023). More is not a better target. Selenium in full blood can add context when the food estimate is uncertain.
